Meet the staff |
Peter D. Cladis, M.D. joined our practice in July of
1983. He graduated Summa Cum Laude from Denison University in Granville, Ohio, and received his M.D.
degree from Northwestern University Medical School in 1980. His residency was served at the Cedar Rapids Family Practice
Residency Program where he was chief resident in 1982. |

Michael J. Fremgen, M.D. graduated Cum Laude from
Marquette University in Milwaukee and received his M.D. degree from the Chicago Medical School in 1982. He completed his residency program at the University of Illinois in Rockford. Dr. Fremgen joined our practice in the summer of 1985. |

Joseph M. Neubauer, M.D. graduated from the University of
Illinois and earned his M.D. from Rush Medical College. He served his residency at the Lutheran General Family Practice
Residency Program. He joined FVFP in July 1987. |

Thomas R. Hazel, M.D. received his undergraduate degree
from Wayne State University and his M.D. degree from Michigan State University. He completed his residency at West Suburban Medical Center and
joined FVFP in 1990. |

Clark A. Zeddies, M.D. graduated
Summa Cum Laude from Alma College and received his M.D. degree from Wayne State University. He completed his residency at Resurrection Medical Center and joined FVFP in July 1990. |

Warren M. Beeh, M.D. grew
up in the Tri-Cities area, received his undergraduate degree from
Northwestern University and his M.D. degree from the University of
Illinois in 1979. He completed his residency in Family Practice at St. Vincent Hospital in
1982. Dr. Beeh joined FVFP in 1991. |

Katherine G.
Fackler-Chapman, M.D. joined FVFP in October 1996. She
graduated Magna Cum Laude in 1982 from Valparaiso University. She received her M.D. degree from Ohio State University in
1986 and completed her residency at R.L. Thompson Strategic Hospital at Carswell AFB, TX in 1989.

Kristin D. Drynan, M.D.
grew up in Yorkville, received her undergraduate degree from
Augustana College and her M.D. degree from Northwestern
University Medical School. She completed her residency at the
University of Illinois College of Medicine, Methodist Medical
Center in Peoria, Illinois. She joined FVFP in October 2007.

Mid-Level Providers
Stacey Brown, PA-C, joined FVFP in 1999. A Summa Cum Laude graduate of Carthage College, she graduated the Valedictorian of her
class at the Finch University of Health Sciences at the Chicago Medical School Physician Assistant master's program.
Mary Beth Kunzer, FNP, joined FVFP in September 2001. A 1996 BSN graduate from Millikin University, she
graduated with distinction from DePaul University in 2001 where she earned
her Master of Science in Nursing, Advanced Nurse Practitioner Degree.
Heather Kluck, PA-C, joined FVFP in January 2005. She is a 2000
graduate from Augustana College with a bachelor's degree in Pre-Medicine
and Biology, and a 2004 graduate from Midwestern University with a
master's degree in Physician Assistant Studies.
Suzanne Nicholson, FNP, joined FVFP in October
2005. She received her undergraduate degree in Nursing from Elmhurst
College and her Master's Degree in Nursing/Family Nurse Practitioner
from Northern Illinois University.
