Clinic Communiqué

The following are responses to frequent questions from our patients. The information may be helpful to you.

New State Requirements for Children - There are two new state requirements in Illinois regarding your child’s health care and entry into school that became effective on January 1, 2008.

  • The first new law states that a physical examination is required for entry into sixth grade rather than fifth grade.
  • There is also a new state law requiring an eye examination for all children entering Kindergarten, or any child new to the district in 2008. This eye exam must be done by an optometrist or an ophthalmologist and will therefore not be included as part of their exam here.

You may contact your school nurse for more information on these requirements and/or to obtain forms.

School, Sports and Routine Physicals - are being scheduled approximately 4 - 6 weeks in the future. Please plan ahead and schedule accordingly.

Saturday Appointments - are limited to emergency care only.

Under Age 18 - Patients presenting for an appointment under the age of 18 must be accompanied by a parent or have a note giving permission to treat.

Late Appointments - If a patient arrives 0 - 10 minutes late for a scheduled appointment, we will fit them in, in the order they arrive. If they are greater than 10 minutes late and we cannot accommodate them into the schedule, we will ask them to reschedule the appointment.

Missed Appointment - Failure to show-up for an appointment will result in a $25 fee.

Cancelled Appointments - We kindly ask for a 24-hour cancellation notice.

After Hours Calls - A charge may result from an after-hours call to a physician.

Prescription Refill Requests - Please allow 24 hours for processing refill requests.

Insurance - A patient without insurance will be required to pay in full upon leaving.

Patient Due Balances - We require payment on the patient due portion of the bill upon checking out.

Copies of Records - Please allow up to a week for processing record copies.

Disability or Other Insurance Forms - Please allow up to 72 hours for processing. There may be a fee depending on the length and time involved to complete the form.
